SOUHRN
Hypertyreóza představuje nejčastější endokrinní onemocnění starších koček a současně významnou příčinu systémových komplikací, zejména kardiovaskulárních a renálních. Článek shrnuje současné poznatky o diagnostice a terapii kočičí hypertyreózy se zaměřením na využití scintigrafie a radioaktivního jódu (131I). Diskutovány jsou možnosti laboratorní diagnostiky, význam stanovení tT4, fT4 a TSH, vliv komorbidit na interpretaci výsledků a přínos scintigrafie štítné žlázy při posouzení rozsahu funkční tyreoidální tkáně, detekci ektopických ložisek a plánování individualizované terapie radioaktivním jódem. Součástí práce je také soubor 19 koček léčených radioaktivním jódem na specializovaném pracovišti v České republice v období únor–květen 2026. U většiny pacientů došlo po jediné aplikaci 131I ke klinickému zlepšení, přesto však výsledky potvrzují význam pečlivého monitoringu renálních funkcí a včasného záchytu iatrogenní hypotyreózy po terapii. Článek současně zdůrazňuje význam otevřené komunikace s majitelem pacienta jako důležité součásti dlouhodobě úspěšné léčby.

SUMMARY
Hyperthyroidism is the most common endocrine disease in older cats and an important cause of multisystemic complications, particularly cardiovascular and renal disorders. This article summarizes current knowledge regarding the diagnosis and treatment of feline hyperthyroidism, with a special focus on thyroid scintigraphy and radioactive iodine (131I) therapy. This review discusses laboratory diagnostic methods, including the significance of total thyroxine (tT4), free thyroxine (fT4), and thyroid-stimulating hormone (TSH) measurement, the influence of comorbidities on interpretation of laboratory findings, and the role of thyroid scintigraphy in assessing the extent of functional thyroid tissue, detecting ectopic thyroid tissue, and planning individualized radioactive iodine therapy. This study also includes a cohort of 19 cats treated with radioactive iodine at a specialized centrum in the Czech Republic between February and May 2026. Most patients showed clinical improvement after a single administration of 131I; however, the results also confirm the importance of careful renal monitoring and early detection of iatrogenic hypothyroidism following therapy. The article further emphasizes the importance of open communication with the owner as an essential part of successful long-term management.*
